Let EnVision Quality Supports assist you or someone you know, in moving from a nursing home to your own apartment, home, residential or assisted living placement. At EnVision Quality Supports, our mission is to provide quality services and supports in a way that is individualized for each person. Our team of caring professionals will:
• Help you determine which services best meet your needs.
• Develop a plan of care that addresses medical, safety and resource issues.
• Coordinate with family, facilities, care providers and Medicaid.
• Regularly monitor services for quality of care and conduct annual assessments to assist you in remaining as healthy and independent as possible.

Eligibility
- Must be 18 years or older
- Must be a nursing home resident for at least 90 days and be medically eligible for long term care services under Medicaid
OR
- Must have resided in a nursing home for at least 30 days on a Medicare-certified stay and be medically eligible for long-term care services at the end of the Medicare stay
OR
- Must have resided in a licensed assisted living facility on an extended stay basis of 265 days or more.
