New Choices Waiver Case Manager
Hailey received her Bachelor of Social Work from Utah State University in 2012, and holds a current license with the state of Utah as a Social Service Worker (SSW). Hailey has a background of working with children and adults with disabilities in foster care as well as elder care in assisted living centers. She loves helping and watching clients achieve their goals and have long term success. Hailey loves being a mother to her two children, spending time with her husband, family and friends, anything outdoors, and going to the movies.
